How to fill in the application form in your International Business Package (IBP).

Welcome to our Herbalife family. We are so excited for you. Have you had a chance to go through everything in your International Business Pack? Take it step by step and always remember this is a fun, simple, magical business.

To start with we have some administrative things we need to go over.
In your IBP (International Business Pack) is the Herbalife Application form.
Remove the card that is attached to the top left of the form. The number that you see on that card is your personal Herbalife identity number, and allows you to deal directly with the Herbalife offices in all the countries!

Fill out the application form with all your info in all the applicant fields. Please make sure that all your details are filled in 100% correctly, and that the application form is signed and dated.

In the upper section of the form you need to complete your personal information. Please print clearly in block letters and press down firmly, so that all the copies are clearly readable.

Last Name (Surname), First Name, Middle Initial or Middle Name

Full Address (If PO Box or different from Permanent Residence Address, then the Permanent Residence Address will be completed further below, in the section provided for that purpose.)

Telephone numbers, including country and city long distance codes.

There is a small print question to the right of the telephone information, regarding authorization to send you information via e-mail. Be sure to mark the appropriate answer.

The date of birth must be completed as: 2-digit day (01-31), then the name of the month spelled out and then the two-digit year. So for example,
A person born on: would complete the form as:
March 25th, 1966 25 MARCH 66
July 3rd, 1954 03 JULY 54

If you and your spouse are both becoming distributors, there is a specific space in the form to enter your spouse’s details

Below there is a box for Sponsor information. Your Sponsor is the person who directly introduced you to the Company. In that space, you complete your Sponsor’s full name, address and your Sponsor’s Herbalife ID number. Make sure you verify with your Sponsor the address you should enter in this box.
To the right of the Sponsor information there is Supervisor information. In some cases the sponsor and the supervisor are the same person. You must still complete the Supervisor information! The person processing your application has no way to know who your Supervisor is, unless you complete all the information. If you don’t know or are unsure, ask your sponsor before completing the form.

The furthest right area in this section is for entering the information about your upline. Please be sure to check with your Sponsor or Supervisor regarding the various levels to complete.

At the bottom of the application write the date and place and sign it.

After completing the Distributor application, place the first two copies into the envelope, which you will find in the IBP box and send to Herbalife.

Keep one copy for yourself and mail the remaining two copies (for Sponsor and Supervisor) to your sponsor and supervisor, respectively. If they are the same person, mail both copies to that person!

The sooner you post them back, the quicker you are entered into the Herbalife Worldwide Computer network.
On the card, write the expiry date as a year from the date on which you sign the agreement ( we renew our distributorships on a yearly basis).
Put the card in a safe place. The number on the card is your unique Herbalife ID number. When you call Herbalife, the first thing they need to know is your Herbalife ID number so they can access your details.
